Thank you for your patience as we work through this difficult time. While we are anxious to be able to accommodate all of our patients, your safety and the safety of our team are our highest priority. Because of this, we will be seeing patients on a limited basis for the time being.
Due to only seeing a limited amount of patients in a day, it is even more important than ever that you call to reschedule at least 24 hours in advance, so that we are able to accommodate other patients in need. We will be enforcing a $50 no show/cancellation fee if you do not give us at least 24 hour notice.
We are doing everything we can to make our office as safe as possible. Here are some of the steps we are taking to maintain a high level of safety:
Our assistants, hygienists and doctor will be wearing a clean gown, mask, face shield, and goggles to see patients. Our other team members will be wearing a mask. We also ask that patients wear a mask as they enter and leave the building. One will be provided if you do not have one.
All patients will be screened for COVID-19 symptoms before their appointment and at arrival for their appointment. We will also be taking all staff and patients temperature upon arrival. Please see the consent below. This must be signed by all patients seen at our office.
All patients are to remain in their car and either reply to their check-in text or call the office upon arrival. A team member will either meet the patient at their car or the door to screen them before their appointment and to record their temperature. They will then be asked to sign the consent form. Only one patient will be allowed in the waiting room to check-in at a time. They will then be taken directly to the operatory.
We will have air purifiers that clean the air 5 times an hour in each operatory. We will have sneeze screens at the reception desk for checking out.

With the extra precautions in place, extra time will be allotted for each appointment. This unfortunately means that we will not be able to see as many patients as we normally would. If we’ve had to re-schedule your appointment, we truly apologize for the inconvenience. If you still need to reschedule your appointment, please call our office to do so.

I knowingly and willingly consent to have dental treatment completed during the COVID-19 pandemic.
I understand that the COVID-19 virus has a long incubation period, during which carriers of the virus may not show symptoms and still be highly contagious. I know that it is impossible to know who is carrying the virus.
Some dental procedures create aerosols, which is how the disease is spread. This can linger in the air for a period of time, increasing my risk of contracting the virus in a dental office.
I confirm with all the statements on this consent and that all of my responses have been truthful. I take full responsibility of any consequences and risks of dental treatment.
